Children’s Services and Adult Services
Interchange Outer East is still experiencing a high number of inquiries for our services and programs.
If you are wishing to access any of the following services/programs:
– 1:1 Support
– Plan Management
– Support Coordination
– Day Service
– Intensives (young adult development and learning)
– Family Support
If you are experiencing difficulty in completing the form please contact IOE reception on 9758 5522 and you will be re-directed to someone who can assist you.
Please note: This is not a registration form and completion of this form does not mean that you are immediately registered to access services.
As IOE’s ability to register new families varies depending on the capacity of any given program, we are unfortunately unable to provide you with a timeline from the submission of your expression of interest to your actual registration. We strongly suggest you continue to seek alternative services in the meantime.
IOE is a family focused service and our services are developed specifically around the needs of children and young adults. For this reason priority will be given to children and young adults living at home with their family.
We provide our core services within Melbourne’s outer eastern region; City of Knox, City of Maroondah and Shire of Yarra Ranges. Although we are open to providing service in other areas, service priority is given to people residing within these municipal regions, as they are the areas in which our support staff predominately live.
We acknowledge that accessing services while transitioning to NDIA can be extremely difficult. If you are finding that linking with services from any agency is continuing to be challenging please contact either your Local Area Coordinator (LAC) and or NDIA planner for assistance.
If you have any questions about the registration or intake process please contact our Intake team at email@example.com
Even though you are not registered with IOE, you and your family can still take part in a number of family support programs and group programs run by IOE – see below.
