Applying for IOE Services

Take the first step towards getting involved in our programs and services by completing the relevant form below.

Children’s Services and Adult Services

Interchange Outer East is still experiencing a high number of inquiries for our services and programs.

If you are wishing to access any of the following services/programs:
– Recreation
– 1:1 Support
– Plan Management
– Support Coordination
– Day Service
– Intensives (young adult development and learning)
– Family Support
please complete and submit our Expression of Interest online form  This is a secure form and all information collected is confidential and will be kept in accordance with Interchange Outer East’s Information Privacy policy.

If you are experiencing difficulty in completing the form please contact IOE reception on 9758 5522 and you will be re-directed to someone who can assist you.

Please note: This is not a registration form and completion of this form does not mean that you are immediately registered to access services.

As IOE’s ability to register new families varies depending on the capacity of any given program, we are unfortunately unable to provide you with a timeline from the submission of your expression of interest to your actual registration. We strongly suggest you continue to seek alternative services in the meantime.

IOE is a family focused service and our services are developed specifically around the needs of children and young adults. For this reason priority will be given to children and young adults living at home with their family.

We provide our core services within Melbourne’s outer eastern region; City of Knox, City of Maroondah and Shire of Yarra Ranges. Although we are open to providing service in other areas, service priority is given to people residing within these municipal regions, as they are the areas in which our support staff predominately live.

We acknowledge that accessing services while transitioning to NDIA can be extremely difficult. If you are finding that linking with services from any agency is continuing to be challenging please contact either your Local Area Coordinator (LAC) and or NDIA planner for assistance.

If you have any questions about the registration or intake process please contact our Intake team at

Even though you are not registered with IOE, you and your family can still take part in a number of family support programs and group programs run by IOE – see below.

Family Services

Family services are open to all families who have a child or young person with a disability. Families do not need to be previously registered or have any affiliation with IOE.

However, as of 1 January 2020, if you wish to use any of our Family Support services, we now require you to complete our new Family Support Services enrolment form, which will provide us with your contact details and proof of carer status.  This will enable us to send you information about what is on and allow you to access activities.

If you have any questions about what we can offer your family please contact us on 97585522 or email

Other Available Programs

We have a number of group programs which all children and young adults with disabilities, plus their families, can attend. It is not necessary to be either on our waiting list or be fully registered with us. Please remember to contact us first to let us know you would like to participate in the group or activity.

To learn more view our Program List. We are constantly developing new programs and adding them to this list.  For more information, or to apply please contact 9758 5522 or
