Registering for the DSR

Print Friendly

Department of Health and Human Services (DHHS) has a register called the Disability Support Register (DSR) where you can register for ongoing supports.

Registering for the DSR assists with an easier transition to the NDIS when the area rollout begins as the DSR can form the basis of your plan. Whilst the application is a time consuming process, having a completed plan prior to NDIS rollout makes the NDIS process easier.

To apply for the DSR, see the documents which are downloadable via this webpage:
http://www.dhs.vic.gov.au/about-the-department/documents-and-resources/reports-publications/application-for-ongoing-disability-support-forms
Choose Application form for Individual Support Package only  – listed under ‘Downloads’.
